First Visit

On your first visit to our office you will be greeted with a warm smile and welcomed. You will then be asked to fill out some forms that will help us gain important information about you and your condition. If you have health insurance you will be asked to provide your card so that we may, with your permission, make a copy for our records and of course to determine your coverage.

Please visit our download page and download and complete the appropriate forms and bring them with you to our office.  This will save you time on your first visit.
